Documents We May Ask You to Provide

Every Medi-Cal case is different. The exact documents depend on the application, program, individual circumstances, and what the eligibility office requests.

Medi-Cal Documents Application copies, renewal forms, notices, requests for information, case correspondence, or other Medi-Cal paperwork.
Identification Identification or other identity-related documents when requested for the case.
Address / Residency Documents related to the applicant's current address or residency when applicable.
