The four things most families want to know first.
Is RASCC free?
Yes. RASCC does not charge the consumer, resident or family a placement or referral fee.
Who makes the final decision?
You do. RASCC identifies and coordinates options, but the consumer or legal decision-maker chooses the provider.
Do we need to sign anything?
Yes. RASCC requires its Service Agreement and appropriate release-of-information authorization before services begin.
Can you speak with the family?
We can have general conversations, but confidential care information requires authorization from the consumer or legally authorized decision-maker.

What does Rosenthal Adult & Senior Care Connections do? +
RASCC helps adults, seniors and their families understand care needs and connect with appropriate care and housing options.
Our core services include:

We also help connect families with other resources when appropriate, including transportation, benefits, household-assistance programs, mobile notary services and independent financial or lending partners.

RASCC reviews a high-level overview of the person's health conditions, physician information, mobility, supervision needs, daily-care needs and other relevant information.
The purpose is to help determine what types of care settings or services may be appropriate and to improve the quality of the matching process.
What types of care can RASCC help me find? +
Depending on the individual's needs, RASCC may assist with options such as:
- Independent living
- Residential assisted living homes
- Larger assisted living communities
- Memory-care settings
- In-home caregiving services
- Other appropriate care resources
If the person's needs appear to require a level of care outside the scope of the providers we work with, we will tell you that rather than trying to force the placement.
Can RASCC help me find care at home instead of moving? +
Yes. If remaining at home is the preferred option, RASCC can help connect you with an in-home caregiving agency.
We look at factors such as:
- Number of care hours needed
- Location
- Budget
- Care needs
- Scheduling requirements
The caregiving agreement is entered into directly between the family or consumer and the selected home-care agency.
What families pay
Is RASCC's placement service really free to families? +
Yes.
RASCC does not charge the consumer, resident or family a placement or referral fee.
When a successful referral or placement occurs, RASCC may receive compensation directly from a participating facility, home-care agency or other partner provider.
Does that mean every service offered by RASCC is free? +
No.
RASCC's placement, matching, referral and related family-support services do not carry a placement fee charged to the family.
Certain separate optional services may involve a clearly disclosed fee. For example, mobile notary services may have a separate charge regardless of whether RASCC is handling a placement.
Does RASCC receive more money if I choose a more expensive facility? +
Referral compensation can vary by provider and agreement. However, RASCC's job is to identify options that fit the consumer's care needs, location, preferences and budget.
The consumer or legal decision-maker remains free to accept or reject any option presented.
Our goal is not to place someone simply because a provider pays a referral fee. The placement still has to make sense for the person receiving care.

How do I get started? +
Start with a discovery call.
During the discovery call, we will ask for a high-level overview of what is happening and identify what type of help may be needed.

Yes.
Before RASCC begins placement, care matching or requests confidential records, the consumer or legally authorized decision-maker must review and sign the required RASCC documents.
These generally include:
- RASCC Service Agreement
- Release of Information Authorization
We require this before significant work begins so that everyone understands the relationship, our role and who has authority to make decisions.
What documents might RASCC request? +
Depending on the situation, RASCC may request relevant medical or care information needed to understand the person's placement needs.
Examples may include:
- Physician reports
- Care assessments
- Medication information
- Hospital or discharge documentation
- Mobility or supervision information
- Other records relevant to determining the care need
We request information only after appropriate authorization is in place.
How long does the placement process take? +
There is no single timeline.
Some cases can move quickly when care needs, finances, documentation and decision-making authority are clear. Other cases take longer because the family is still comparing options, medical information is incomplete, funding is pending or the legal decision-maker needs additional time.
RASCC can help organize and move the process forward, but we will not pressure the legal decision-maker into making a care decision before they are comfortable.
Who RASCC can speak with
Why does RASCC ask who the Power of Attorney or legal decision-maker is? +
Care placement can involve sensitive health information, financial information and decisions about where someone will live and receive care.
RASCC needs to know who has legal authority to make those decisions when the consumer is not making them personally.
Identifying the legally authorized decision-maker early helps prevent confusion, conflicting instructions and unauthorized disclosure of confidential information.
Can RASCC speak with adult children, siblings or other family members? +
RASCC can have general conversations with family members, referral sources and other concerned parties.
However, confidential care information and private records are discussed only with:
- The consumer
- The legally authorized decision-maker
- An individual who has been properly authorized to receive the information
Being a family member by itself does not automatically give someone authority to receive confidential information or make care decisions.
What if the consumer does not have a Power of Attorney? +
A Power of Attorney is not automatically required in every RASCC case.
If the consumer is able to make and authorize their own decisions, RASCC can work directly with the consumer.
If another person is making decisions on the consumer's behalf, RASCC may need documentation establishing that person's authority before confidential information is shared or services proceed.
How we find options
How does RASCC decide which facilities or agencies to recommend? +
We look at the entire situation rather than matching someone based on one diagnosis.
Factors may include:
RASCC then develops a focused list of options that appear capable of meeting the complete need.
Does RASCC guarantee that a facility will accept my loved one? +
No.
Each facility or provider makes its own admission or acceptance decision based on its licensing, available services, staffing, care capabilities, current availability and the information presented during its own assessment.
RASCC can improve the quality of the match, but we cannot guarantee admission or acceptance.
Am I required to choose one of the options RASCC gives me? +
No.
The consumer or legally authorized decision-maker remains in control of the decision.
You may review RASCC's recommendations, tour or speak with providers, conduct your own research and decide which option you believe is best.
RASCC provides the connection. You make the decision.
Do I sign the care or residency agreement with RASCC? +
No.
RASCC has its own service agreement covering our placement and referral relationship.
If you choose a facility, home-care agency or other provider, the applicable residency, caregiving or service agreement is entered into directly with that provider.
RASCC is not the facility, landlord, home-care employer or direct-care provider merely because we introduced you.
What if a physician says my loved one needs skilled nursing? +
Tell RASCC immediately.
We may review available physician information and help clarify what type of care setting should be explored, but RASCC does not override a physician's medical judgment.
The receiving facility must also independently determine whether it can legally and safely meet the person's needs under its license and scope of services.
Paying for care
Can RASCC help if we are applying for CalAIM, ALW or another government-funded program? +
RASCC may help with resource navigation and identifying providers or facilities that work with certain government- funded programs when appropriate.
However, RASCC does not control whether a government agency, health plan or program approves or denies an application.
Who pays while government funding is still pending? +
Unless the selected provider has agreed in writing to a different arrangement, the consumer and/or responsible party should expect to be responsible for costs that are not yet covered by an approved funding source.
RASCC does not pay the facility or reimburse the family for care costs while a government-benefit application is pending.
The exact payment obligations should be confirmed directly with the selected facility, agency or provider before admission or service begins.
Can RASCC help us look for other household assistance? +
Yes.
When RASCC is assisting with placement, we may also help identify programs worth exploring, including:
- Utility-bill discounts or assistance
- Property-tax exemptions or assistance programs
- Food-assistance programs
- Cash-assistance programs
- Subsidized phone service
- Other government or community resources
Eligibility is always determined by the agency or program administering the benefit.
Can RASCC help with home equity or reverse-mortgage options? +
RASCC may connect homeowners or families with independent lender partners who can discuss possible home-equity or reverse-mortgage options.
RASCC does not make lending decisions and does not guarantee approval, rates, terms or eligibility.
More than placement
Does RASCC offer mobile notary services? +
Mobile notary services may be available for documents that require notarization at a residence, hospital, assisted living home or other appropriate location.
Mobile notary service is separate from RASCC placement services and may carry a separate fee.
Can RASCC help coordinate transportation? +
RASCC may help coordinate appropriate third-party transportation for appointments, facility moves, hospital discharges and other scheduled or time-sensitive non-medical transportation needs.
