Care Needs Assessment

We will review a high-level overview of your health conditions and care needs based on your physician's report and other available care information.

GOAL Understand what level of care and support may be appropriate.

Assisted or Independent Living Home Placement

We will find care and housing options that best match your preferred location, budget, care needs and personal preferences.

GOAL Give you a focused shortlist instead of making you search hundreds of facilities yourself.

In-Home Care

We will connect you with an in-home caregiving agency that matches the number of care hours you need, your location, budget and care requirements.

GOAL Find appropriate caregiving support while allowing the individual to remain at home.

Family Support

We will keep our communication lines open so you can contact us when questions, concerns or additional needs come up during the care and placement process.

GOAL Make sure your family does not feel abandoned after the initial referral or placement.
Care Needs Assessment RASCC's review is intended to help with care coordination and placement matching. It does not replace a physician's medical diagnosis, medical advice or a clinical evaluation by a licensed healthcare professional.
YOUR PATHWAY WITH RASCC

What happens after you contact us?

We want every family to understand the process before care information is collected or a referral is made.

IMPORTANT: CONFIDENTIAL INFORMATION Protecting the consumer's private information comes first.

RASCC may have a general introductory conversation with a family member, hospital, referral source or other concerned party. However, sensitive care information and confidential documents will only be discussed with the consumer, the legally authorized decision-maker — such as the Power of Attorney — or another individual who has been properly authorized in writing.

STEP 02
LEGAL AUTHORITY

We establish contact with the legal decision-maker.

If another person is legally making care decisions for the consumer, RASCC will establish direct communication with that individual, such as the Power of Attorney or other legally authorized representative.

We will explain our services, how the referral process works and what RASCC can and cannot do on the family's behalf.

STEP 03
AUTHORIZATION

Review and sign the required documents.

Before RASCC begins care matching or requests confidential records, the consumer or legally authorized decision-maker must review and sign:

RASCC will not begin placement or referral services until the required authorizations are completed.

STEP 04
MATCHING

We build the complete picture.

Once authorized, RASCC may request relevant records from medical providers and other appropriate sources.

We look at the entire situation — not just a diagnosis.

RASCC then creates a focused shortlist of facilities, services and/or in-home care agencies that may fit the complete need.

STEP 05
YOUR DECISION

You choose the option that works for you.

The consumer or legally authorized decision-maker remains in control of the final decision.

You may review the options presented by RASCC and decide which facility, caregiving agency, service or provider you want to work with.

Any admission, care or service agreement is entered into directly between you and the selected facility, agency or provider.

MORE THAN JUST PLACEMENT

We may be able to help with more than the care search.

Care transitions often expose other financial and practical needs. When we are assisting with placement, we can also review several areas that families frequently overlook.

COMPLIMENTARY WITH PLACEMENT

Utility Bill Discounts

We can help check whether you may qualify for utility discounts or assistance programs based on factors such as age, income, disability status or household circumstances.

COMPLIMENTARY WITH PLACEMENT

Property Tax Programs

If you own your home, we can help identify property-tax exemptions, exclusions, postponement programs or other assistance that may be worth reviewing.

COMPLIMENTARY WITH PLACEMENT

Government Benefit Review

We can help identify government or community programs that may be available, including food assistance, cash benefits, subsidized phone service and other household resources.

SEPARATE FEE APPLIES

Mobile Notary Services

Mobile notary services may be available at a residence, hospital, assisted living home or other appropriate location. Notary fees apply separately from RASCC placement services.

INDEPENDENT LENDER PARTNERS

Funding & Finance Connections

When appropriate, we can connect homeowners with independent lending professionals who can discuss home-equity options, including reverse-mortgage solutions.

COORDINATION SERVICE

Transportation Coordination

We can help coordinate non-medical transportation for appointments, hospital discharges, facility moves and other scheduled or time-sensitive transportation needs.

Eligibility for utility discounts, property-tax programs, government benefits, financing and other third-party services is determined by the applicable agency, provider or lender. RASCC does not guarantee eligibility, approval, savings or financing terms.
OUR PROMISE TO FAMILIES

You do not pay RASCC for placement services.

RASCC will never ask the consumer, resident or family to pay RASCC a placement or referral fee.

Our placement, matching, referral and related family-support services are provided without a placement fee charged to the consumer or family.

When a successful referral or placement occurs, Rosenthal Adult & Senior Care Connections may receive compensation directly from the participating facility, home-care agency or other partner provider.

You will sign any applicable residency, caregiving or service agreement directly with the facility, agency or provider you choose.

Separate optional services: Certain services that are not placement or referral services, such as mobile notary services, may carry a separately disclosed fee.
